I don't know if this is a bug or unfinished content type stuff.

But during my game I have come across like 25 things that have the show the little "gears" when I put my mouse over them. I obviously click on them because that means you interact with them.

But some things have no dialogue or investigation and my characters have no reaction to it.
It looks like a bug to me.

Here is an example just now.

I am in the prison at Wyrm rock there is the "unusual skull" to click on in one of the prison cells. I click on it but nothing happens at all.

So I went and typed in BG3 unusual skull on internet and found a video that shows this skull saying a riddle that we need to solve....

So I am not getting any riddle because there is nothing happening when I click on it.

Now I wonder how many other things did not react or work when I clicked on them.

I wonder how much content I am missing because of this type of bug.

The amount of content any individual player misses on a single play through is most likely quite large. Some content is just branched away from based on decisions related to other content.

In this case, the content you are seeing has not yet been 'triggered' by whatever needs to happen to make it interactive. Generally speaking, these are not bugs (but it is hard to say definitively though).

Some of these things I've noticed may be due to an ability check not triggering or it does trigger but doesn't show if you fail, so you didn't know if one actually happened. Or at least there's been one or two times where I've clicked on something as my main PC but then clicked it again as a companion and they had a check that succeeded and something happened.

Yes, the skull reacts if you get imprisoned there. If you just break in the skull has nothing to say.

Many things are based on who your companions are at the time and the choices you make so you will not see everything in a single run.
