Girdle of masculinity/femininity question
Real talk. In D&D (and in the other Baldur's Gate games) there is an item that reverses your biological sex. It's called a girdle of masculinity/femininity. What happens with it when a transsexual PC wears it? If they have female breasts and male genitals, do both just swap? Same thing in reverse? What about ovaries, menstrual cycles and the ability to bear children? Does it just do nothing? The item is cursed, so I feel like it should do *something* and that something should be bad for the player.

Well, I think we can imagine some possibilities
I think it makes sense that the hole body will change, not only the genitals, right?
So the breasts wouldn't matter, if you change to female you'll keep the breasts/gain new ones

Biological sex is simply groups of traits over a dimorphic curve - so it would depend on what the magic is actually working on whether it'd change something

In general though, transgender people are trans-gender

If they wear it they'll become cis but presuming it works on surgery and hormone production, they might need surgery or need to quit HRT

Also, if you transformed by putting the belt on, what would a DNA test say about you with regards to your original form? You can't be a clone, but you also wouldn't match a sibling profile. WHAT WOULD IT SAY? ಠ_ಠ

it would probably say you're afflicted with Confusion and to seek help at a temple.

:BatteryFull:I haven't played D&D 5th edition yet,
but from what I've been told ..Pre-op Transsexual Player Characters (PCs) would swap genitals and lose or gain a g-spot.
The girdle of masculinity/femininity allows for pregnancy and menstrual cycles without any need to roll for stopping bleeding. :DDLuck:

The notion that the item is cursed is an out of date mechanic that is considered triggering in many D&D groups. These days we handle the girdle differently than back during the earlier D&D versions.

That said, the girdle does not change the brain of the player character.
A lesbian, non-binary or cis-curious queer epic D&D hero character would still lust for the same non-player characters and player characters as usual, and are still subject to other magical spells and items as per the official manuals.

Many progressive house-rules D&D groups leave out these aspects of the game as they are highly triggering and cause huge disagreements and arguments in the D&D groups.

Just as player characters are not required to roll dice :DDLuck:
every time a character poops, pees and masturbates. It is easier to play Dungeons & Dragons without these aspects cluttering the gameplay.

I have only been in two separate D&D groups where the players are required to state when their player characters are going to take downtime to pee, poop and masturbate.
And since one of the groups had a longtime biologically female identifying as a female, she was exempt from describing said actions after her threatening to quit the D&D group.:BatteryFull:

If an item changes a character's biological sex then I guess it would change their biological sex. I don't see what the question is. XX to XY or whatever.
