[SPOILER] What's up with Duke Stelmane?

I don't understand fully what's up with the Emperor's story and the Duke. He first tells us that she was his ally and they ran the Knights of the Shield. We later see the Emperor revealing that he's mind-controlling the Duke. We also learn from some medical notes in the Emperor's basement that the Duke condition (the stroke Wyll mentioned I believe) improved upon the visits of the stranger (Emperor). Was she controlled against her will as the Emperor's puppet or his ally?

There is a lot of stuff I didn't fully get about all that relationship. It sorta seems like he had genuine feelings for her, but he is a manipulative mindflayer so I don't really know what is true.
There's a lot to the Emperor that is revealed over time if you do all the side stuff and it keeps me flip flopping on if I trust/like him or not xD

I just went through his chambers again on my second playthrough and met him in the dream again and rejected him when he made advances. He tried to show to me the ''truth'', which seems like he was controlling her against her will the entire time and called me a puppet. So I guess he was just mind-flaying around
