Confront Lord Gortash (bug)

I've already did the whole thing without kalach's. Yet the quest won't update, and now I'm soft locked and the game won't let me progress into other regions. ( Can't reload an old save to far ahead and have done every other quest in lower city)

Keep getting "A member of your party is not ready to proceed" So I'm stuck in Lower city.

Tried sending karlach to prison, doesn't work, killed her doesn't work. She's completely bugged out and won't let me progress so i can't kill Gortash.
