I don't know how it plays on a good aligned playthrough but before his quest astarion would straight up block the twins option completely and when resolving Astarion's quest in the Evil way I was able to get the 5some. Again, no idea what happens if you resolve the quest another way as I wasn't romancing him on my good aligned playthrough
idk if there's a way to avoid it but in my case she lied ;) eventually she's like "oh you like bae'zel that much huh, well i wanted ALL of you so good luck with her!" or something like that.
