I don't know how it plays on a good aligned playthrough but before his quest astarion would straight up block the twins option completely and when resolving Astarion's quest in the Evil way I was able to get the 5some. Again, no idea what happens if you resolve the quest another way as I wasn't romancing him on my good aligned playthrough

You have to already have the kind of threescore going with Astarion and Halsin. Then have em both there. The twins still only show interest in you and Astarion but Halsin invites himself along and they don't object.

idk if there's a way to avoid it but in my case she lied ;) eventually she's like "oh you like bae'zel that much huh, well i wanted ALL of you so good luck with her!" or something like that.
