I killed him for the achievement by killing all the nurses first. He spent every turn just trying to revive them since he can't fight without them.
But i'll guess you need to rid of nurses first of course (which you can still engage in battle with him alone after successfully convince him let nurses "practice" on each other then fail the second part of convincing him to sacrifice himself to show true loyalty to dark goddess Shar).
I'd imagine supposedly surgeon is a skill cast by the Malus Thorm when nurses are active because they each wield a different surgery tool and he may order any of them to pass over their tool to him and then he hops close to you or your party member and prone you to do operation, something like that.
The most annoying one has got to be the money boss though. He just one shots whoever carries the purse strings
(So I dropped the money, then he started using the other 12-100 gold on the floor to summon monsters. I started being afraid and drag drop my 12k further away from him)

I think that's the point. The Thorm Siblings are all flavor and no function if you know what their gimmick is.
