Raising his approval didn't do anything at all, in my case.
I did the Raphael quest exactly as Astarion wanted :) and yet...
As some people are saying in their threads, Astarion will romance you by himself, even if you don't want to.
I hate to say it but yeah, the Drow at Moonrise is actually important to continuing the relationship aspect with him. It is your chance at a second romance scene that continues. Asking Raphael for help with Astarion's scars is integral to his story further.
The end of Act 2 is when a lot of story bits hit a note with most companions that your relationship with them begins to influence alot how their story shapes up. Its also when you need to choose whom you are staying with and are locked to that 1 relationship.

LOL Okay mate. s'alright, you will live and still be considered a true-real man even if another one hits on you. Since you have the power to say no and turn him down in all sort of insulting ways! Your kind have all the tools they need to destroy this character, yet you insist on coming to steam and whining about it is all you can do? So weird, like Larian included a lot of cruelty you can do to him explicitly for the empathy-impaired types.
Yes, I see. But my pickle with the game design is that, while you will always get to meet Raphael and decide what to do, I was not really given the chance to interact with the Drow. She was hostile due to my game path. They should not put such a fundamental interaction in a story part that can be easily unintentionally skipped IMHO. I hope they make a mod to correct this...
