(SPOILERS) There is a way to save Karlach
I've read time and time again that there is no way or happy ending for Karlach. BUT, I just finished my game and found a way that Karlach might live and might find a way to cure herself. It seems that if Wyll becomes the blade of avernus she can take Karlach to avernus and save her from exploding. So if you want to save Karlach withouth transforming her into a Mindflayer you could try this alternative for her.

And this also blocks the final romance dialogues with other characters for some reason if you do follow through with sending her to Avernus. Guess cutting her good ending scripts has messed up quite a few things in the ending.
She said so many time that she prefer to die than return to avernus. I see no happy ending making her return to the place she hate the most.
I cant throw Wyll under the bus for her, she gets turn into a Mindflayer.
It will be possible to save her in a future patch. Community Update #24

my karlach does 75 damage almost every hit, sometimes does 100 you must not have her built correctly

Well if you cleared the house of hope and left hope in charge, there's not too much of a reason to think you're not just all playing Hell family in that mansion

This could be a Head cannon, where she goes back to the House of Hope when she needs a rest.
It's hard to believe in a game based on a system that has a resurrection spell that brings you back to life with your parts all back to normal that there isn't any option for doing that for Karlach. Heck, there's already that divine intervention mechanic for clerics, where's the option to burn that to fix her? Or any magical anything option lol.

Also it bugs me that she's burning up because her surroundings are... no longer as hot? I don't even think they really handwave that away with anything although there might have been something said, but in my head it's just like "hey this oven inside me is getting hotter because it isn't surrounded by another oven anymore" and it doesn't follow.
I was reading this thinking "Has thou the Gods has spoketh?"
Only to find out my hopes were played like a damn fiddle!
The morale of the story is "Life isn't fair."

Had you paid attention in the cutscene where the nautiload ship is attacked by dragons, you'd have seen that there's ice and snow everywhere in avernus. Hell in bg3 is not a hot place, it's a cold place....

The most you can hope for with Karlach is if you didn't romance her and you got both upgrades - then the game ends with no mention of Karlach so she might be alive.

I didn’t romance her, I also got both upgrades. She exploded into flames in the end and died.

The game, like divinity, is extremely complex with its ending alogarithm, and this early likey includes some bugs, so it’s really tough to nail down 100% of the “x causes y ending” figured out.

I believe Wyll needs to become blade of avernus for her to live and go back into the hell's with her.

Although my question is can Wyll become blade of avernus with breaking his contract? If no, then the logical choice is have Karlach become a mind flayer if she's going to die anyway unless she is romanced

She can also turn into the Mind Flayer that defeats the Elder Brain. She retains her identity and is "cured" from her infernal engine. Not an ideal outcome, but it felt better to me than having her live in the hells.
Becoming a mindflayer is a negative outcome. You lose your soul and eventually you lose all sense of your old morals and almost always lose your personality.

Becoming a mindflayer essentially means replacing your character with a (temporarily) friendly version of your parasite evolved into a full being.
