I don´t personally like to play evil characters… So I don´t do it, but there are a lot of people who enjoy playing evil characters and they have fun while doing it.
So do you enjoy of being good, neutral or evil? Go for what you like to do! There is enough to do with all options.
Acting like the story isn't content. :P
It changes a lot. For extra unique evil playthrough do the dark urge origin for your character.
Is it worth it? I guess so, you paid for the game so you may as well get your money worth and experience all it has to offer.
If anything would seem like the game would be a hell of alot less challenging. Seeing that most of the strongest bosses are likely the Evil ones.
im currently on a "good" paladin playthrough and expected a lot of changes if i always help the opposite faction everywhere.
atm evil playthrough is complete crap, as if the developers of the game wanted to tell you F$%^ Y$% for being a ♥♥♥♥, good part of the quest are dump to the trash can and you get in return only Minthara with dont havy even any quest for her, i expect something really good like renegat path in mass effect series but in BG3 evil playthrough is 2/10 just disappointing for missing so much quest.
I also chose "the urge" for a second playthrough but other then slaughtering a certain NPC in my sleep in camp and having to decide what to do with the body before the party members see it, and some extra lines in most deciding dialogs urging to massacre and killing there isn't much difference. But i have yet to discover where it leads.
I think being "evil" just makes you more popular with certain party members that usually disapprove when you do good. Minthara is not one of them though.
It changes a lot. Absolutely worth it. The Dark Urge so far is one of the biggest surprises.
