I originally declined Raph's deal, met Voss in the tap room, then went to House of Hope(HoH) and stole the Hammer. On going to the sewer to meet Voss, I encountered the aforementioned bug.
Then I tried reloading before going to HoH, but well after declining the deal and talking to Voss in the tap room. I went back to Raphael accepted the deal, but Voss was not in the sewers, neither was the dragonborn.
Then, I reloaded to a point before the whole quest, accepted Rapheal's deal when he first proposes it, and then went to meet Voss in the tap-room with the Hammer in tow, and I was finally able to complete the quest and get the reward.
So currently, the only way to complete the quest properly and get the reward, is accepting Raph's deal when he first offers it, immediately going to Voss in tap room, then going to HoH and grabbing your contract and killing Raph. I guess if you don't finish this bugged quest, but free Oprheus regardless, Voss may show up at the end; as the game can auto resolves some quests. I haven't reached the end, so I cannot personally confirm.
- Left Raphael's deal hanging, didn't accept or reject it.
- Went to the House of Hope, got the hammer, beat Raphael.
- Came back to BG, Voss not in Sharess' Caress nor in the Sewers (his dragon is there in its dragonborn form though, for all that is worth).
- Carry on with the Confront the Elder Brain quest, get owned by Elder Brain. Emperor nabs you and brings you back into the Astral Prism. This is where it branches out: insist on releasing Orpheus (comes up twice in dialogue). Emperor will abandon you in the end. Smack the crystals with the hammer, release the Prince. He or you will self-sacrifice to become illithid. Return to the city in ruins, you'll be waylaid by Voss. Orpheus will defuse the situation and the quest will resolve.
It's very long-winded and doesn't seem to be the way it would naturally work but it somehow does in the end.
I've had to reload back 20-30 hours of progress just to get outcomes that I'd like, mostly due to bugs and some stupid game mechanics. It sucks being a completionist and trying to be good and get the best outcome in this game. Act 3 is a buggy mess, so many quests have bugs and broken interactions, and the whole final leg of the journey feels unfinished and rushed as hell.
Sigh... I'm just going to continue to finish the game; this was the last side quest left for me. Only the final confrontation remains.
