Bust inside Devils Fee (Potential spoilers)
Read below -- (couldn't find a spoilers button?)

There is a bust inside Devils fee that you can rotate. I can't get whatever it's supposed to trigger though, and couldn't find any guides or info on reddit.

Anyone know what the bust is for? It's the one behind the trader.

Same here, couldn't figure out what it does. There is a similar bust that you can't interact with in one of the balconies upstairs, but couldn't find a link

Theres a Hatch next to the lady, to the right of the bust. turn the bust 3 times and the lock on the hatch will be open. I tried to get inside, but seems like she wont let you. maybe you have to fight her? she spawns a LOT of enemies

Quick Update: I used Fog, stealth everyone, rotated the bust, clicked the Hatch. no encounter :)

Fun fact...to get down in that hatch, you actually don't even need to go through the Devil's Fee at all. Go to the Forge of the Nine (where Dammon is), on the ground you'll see a door basically to the basement of that same building. Once inside, loot and do whatever, but then look at the rather obnoxious stack of crates. This will reveal a broken wall. Go through that, and voila, you are in the basement of the Devil's Fee. You can actually go upstairs and she won't react to the fact that you somehow got into her place like that.

I cannot find this alleged door on the ground at the Forge of the Nine. :/

So once I found this way in by accident, right before you go upstairs and pop up next to Helsik (who does not mind you coming up but does not let you go back down. Anyway as I was saying, right before you go up those stairs there is a LEVER on the wall to the left of you if your facing the staircase....anyone know what this does???

There is an alleyway that runs north/south on the main map, the north end being the location for the cemetery. The door you are looking for is a locked door, located in the eastern wall of this alleyway.
It's not in the actual Forge, it is outside and in the western wall of the building.

This lever unlocks the hatch from the inside. If you enter the basement via the Forge of the Nine entrance and try to exit via the hatch without having unlocked it, you'll be blocked. Flipping the lever unblocks it, allowing you to use the hatch as an exit.

If you enter from the Forge Of The Nine and explore the Devil's Fee basement, you find all the info you need to automatically let Helsik know that you know that she is legit. Otherwise you have to pass some arcana checks while wandering around the shop.
