Ceremorphosis achievement bugged?

So, basically, I went full squid and i didn't unlock the achievement. Did I miss something?

If you're talking about achievement called "Ceremorphosis", then you should get it when game ends, along with other game ending related achievements.

I finished the game. Albeit, I chose the Bhaal ending while being a a squid. Maybe because of that?

Quite possible - I finished the game without Bhaal, and got the achievement.

Yup.

I had a save before ending and I chose a good ending. It did unlock.

So, you can't have a Bhaal ending + a space squid ending.

I did the transformation, but siezed control. It did not provide the achievement.

I let Karlach become a squid, and it unlocked for me.
