If you want his approval for an action, it needs to either benefit him (a reward, or directly relevant to his interests), or at least be funny to him.
Highlighted by the fact; out of all the companions he's the only one who actively says "disapproves" when you reject his calls for a booty session.
But what's with all the active disapproval?
Because every time you stop to help a kitten out of a tree or whatever, that's time being spent on things that AREN'T Astarion or Astarion-related, and he's enough of a little prick to actively disapprove if you do anything that doesn't benefit him somehow.
He's a vampire who cares what he approves of? Smite him.
