More painful than being smashed in the face with a greatsword or whatever - and in fact, so painful that it breaks the standard rules by allowing NO save whatsoever?
well the check is if you got knocked prone or not :P
i know if im tripping over my concentration is instantly gone and focused on breaking my fall so i don't injure myself. might not be 100% 5e rules but it makes sense to me.
The other option is to take damage and roll a constitution check.
So the only way to break concentration on prone is if the action that made you prone also dealt damage and you failed the check.
Anything other than that is houserule.
