Nope. Whether or not you become a mindflayer comes down to a single choice in Act 3 and has nothing to do with whether or not you've indulged in the tadpole powers. You can be full on illithid and still just not do it and get the "Good" ending.

You also appear as a mindflayer in the cutscene and get a neat little animation of your transformation which looks pretty damn painful
