It's possible that the achievement is only granted for finishing all three acts on Tactician, which is rather disappointing.
How does that work? and what rules is it to trigger these achievements?
Like, can I switch to normal outside combat? or do I have to start with Tactician mode and never change until the end?

I have this achievement, and there are a few things I can note. I started the game on tactician, earlier on I played with a save branched where I was a lower difficulty so I could cheese that fire sword you can get early. I did that, but decided to rollback to before I lowered the difficulty. I then played through tactician without **ever** switching to a lower difficulty. I still got the achievement. From everything i'm seeing online, it also seems like there is a flag whenever you lower the difficulty. Without rerolling to before you did this switch you won't be able to get the achievement. You also need to finish the Act 3 ending all the way through. There are some decisions you can make that would give you a "gameover" ending but I imagine those wouldn't count. So in short, don't ever switch at any point in your tactician playthrough and play through to the very end of the content for critical hit. No skipping final fight for a gameover ending. Hope this helps.
