Why would anyone willingly worship Shar?
OK, so we all pretty much know Shadowheart has probably been brainwashed, since she's way too nice and sweet to be a Shar worshipper (no spoilers, I'm only in act 2), but what about those who follow willingly? We can see in act 2 what Shar worship actually does to people. The hospital, which was more torture chamber than hospital, the entire curse that kills people indiscriminately. There was a story about 2 rings, where a Shar cleric married her lover just before battle, which in reality she just did to give him a cursed ring that ended up killing him to heal her.
Like, this doesn't sound like a good religion even if you're evil and want power, because all it does is destroy. What's there to rule, if your goddess basically just wants to ♥♥♥♥ you over and make everyone (including her own worshippers) suffer, because she's a sadistic ♥♥♥♥♥?
Even Lolth seems almost reasonable compared to that, she gives off more "might makes right" vibes, which makes sense for an evil power-hungry person. But Shar? She just wants to destroy because she hates everyone. Or am I missing something here?

The Hospital is not the average Shar experience, and so arguably is the Shadowland.
Don’t misunderstand: Shar cult is quite a slippery slope of unpleasantness, I completely agree, but the Shadow Land are the cursed place of this story (trademark).
It’s a land that has seen multiple conflicts, where one of the most faithful warriors of a goddess abjured his faith (for reasons we can be in theory sympathetic with), sworn fealty to another goddess (Shar), who cursed the entire land and finally betrayed Shar too in the end, who being a petty entity decided to double down on the curse. Astounding move.
In the meantime, devils and clerics of Shar fought for what was left in the underground, messing things up even further.
You’re looking at the aftermath of tragedies, layered up like a misery cake.
And yes, the people inhabiting the darkness went crazy in the meantime. But how could have be different?

And ehi, there is still so much worse to see!

Wouldn't it be nice to just...forget about your trauma? To feel no more fear, no more anxiety, no more pain or regret?

To get out of the glaring lights exposing every flaw you have to those around you and just rest under a blanket of quiet, comforting darkness?

That's how Shar gets you. The first hit is free, the bill comes due later...

Shar is for edgy 12 year olds and goths.

Baal at least is easy to understand, he's the patron god of murderers and assasins.
