Good for you. I'm playing solo.
That fight definitely has sense of desperation with the massive amounts of enemies spawning, but that also means they're incredibly weak to aoe damage. My personal group consisted of Warlock (main), Shadowheart as cleric, Gale and Karlach. Warlock Eldritch blast with the knock back evocation (or whatever they're called) helps push back 2-3 enemies every turn, kept him and Karlach on guard duty on the ramp leading to the portal while Shadow and Gale lobbed fireballs like maniacs. Shadow also did her Channel Divinity thing once or twice, and I think I summoned an earth elemental with Gale for added meat shieldage.
This was on balanced, so your mileage may vary a lot if you're playing on tactician ofc.

Casting darkness on the portal stops all ranged attacks. Then spirit guardians where the enemies have to climb up. It took a grand total of one hit for me with those tactics on normal.
Hold up. Are you playing solo, or with the NPC team? Because if you're literally solo, just your character, then uhhh.. There's your problem, I wager.
And if you have the NPC team but you're too low-level to learn Wall of Fire, a 4th-level spell (so Gale can learn it at level 7), have you considered.. I don't know.. Leveling up and returning to that encounter later? Not like you have to do it nownownow.
I cleared it fairly easily on the middle difficulty with Gale as the MVP using Ice Storm (another 4th-level spell), and Shadowheart to whomp anything that got close with Spirit Guardians.
My paladin was mostly just an armored back-up dancer, and Astarion (who I'd forgotten in the party because I had something or other to do with him just prior and hadn't remembered to swap back out) barely did anything.
Took me 3 tries on Tactician. Because enemies try to rush portal and you cant heal the portal you need to not let melee enemies to reach it, and ranged enemies to reach the range there they can shoot at it. Various AoE spells help a lot, fire deals massive damage to them. There is also "wall" spell to simply not let them reach the portal.
The easiest way we have done that was by blocking the path for all the undead warriors coming down the road using spells that block movement (like Evards black tentacles, plant growth, spike growth etc.) and repel undead on shadows, that came from the lake. Later on it was just a matter of correct usage of aoe spells, like ice storm, fireball, hordebreaker and couple of granades. Took us like 20 minutes to get over with it, but DAMN that was satisfying. Especially that we achieved it with not one blow to the portal.
edit P.S.: oh yes! Darkness arrow in the portal help a lot too!
