BUG? revive Lae'zel

Hello, for some reason, Lae'zel was dead in the battle against the Githyanki near of col of the mountain. Unfortunately, i can' revive her, the lich don't want to give me the diologue for revive companion, the ressurect ticket and the cleric spell doesn't work.
Anybody can help me please
