Report a translation problem
1) Voice acting. All dialogs are voiced, so you would have added tons of lines of dialogue (one for every npc) that would all need to be translated in all the languages they translate it to.
2) There is no actually good reactions to being naked that would actually be "realist". I mean what games generally do is that they say a line like : "go put some clothes on" or whatever. Which would most likely not be the end of it in reality, and they didn't want to go into the trouble of figuring out how to handle referencing that you're naked just for a little fun.
3) Why block people from playing the way they want to play? If people want to play naked they don't want to be blocked by the game because the lines have all been changed to "put some clothes on" until you dress back up.
4) If you really want a narrative justification, well consider, like many other have mentioned in this thread, that the people of Faerun aren't that prude, and they really don't mind that much if someone runs around naked.
Overall I feel they didn't do it, because of the development cost and because there is no real point to it. If someone wanna play naked, they just play naked. The game doesn't react to any other way you might be dressed, like wearing the armors and clothes of their antagonists or the likes, which I feel would create bigger issues with most than someone being naked.
What's scarier to you? A flaming 7 foot tall creature standing near your house or a visible nipple?

That would at least make this stupid dumb and annoying mechanic now immediately funny and essential to game enjoyment. As I've said many times, this game is just poorly thought-through.
It would go from "hostile game design decision that only serves the purpose to punish the player for making good decisions or ruin their enjoyment of their favourite class" to "I love how immersive these NPCs feel, genius move, Larian!". Unfortunately, we're not dealing with geniuses over at Larian studios.
