Randomly losing actions and bonus actions

Seems like in the first couple rounds of combat I often randomly don't have an action or bonus action. Anyone else having this problem? Is it a bug? Am I getting effected by some invisible condition?

Could be a bug, but check your actions and see if there is not a red "! "Next to it, when I was fighting Cazador I had a effect that prevented me from doing some actions like shove or throw a potion.

I've lost a couple of actions recently when trying to pick items to throw. It seems like it just cancels what I'm doing, uses the resource and burns the action. I was also completely operating on autopilot expecting an outcome so I'm not sure if I just fudged up everything along the way.
