To be fair though, that wisdom roll is really low to keep getting critical failures I am sorry for your lack of RNGesus
After reading your post I tried again and I rolled a 16...wtf is this odd lol...
Yeah I had that happen to me during early access. I had already done a play through where I rejected, and killed her, so there are actually several places where you can add her to your party if you miss her.
Do you really always have a critical failure dice roll?
What class do you play? I think either Intellect or Wisdom is required here and both shouldn't be too low (try to aim for 10, but I made successful rolls with an Int 8 / Wis 16 Cleric)
The game makes me somewhat savescumming in critical situations just so I can try out different solutions. Usually a single second try solves that issue and a better roll helps.
Maybe I got some help 'though by the "story mode" I picked. Simply want to enjoy the story now since I'm no veteran DnD player and don't have B5 ruleset at hand.
That's bad luck. Maybe you shouldn't enter that "conversation" right after reloading your game. Wander a bit around, let some time pass and try it again. Maybe the RNG simply needs more time to give you a better roll.
As I said, usually I can solve any issues with exactly one reload. I save frequently - old habbit of playing computer games for decades.
