When I played, the second I went inside the library in the grove, shadowheart freaked out. But then in the conversation following you can reassure her that nothing will happen if you stick together, so she doesn't show fear after that
Still weird that she's not afraid of gnolls or hyenas.
This is where the cut scene occurred for me.
so maybe its conditional. or easy to miss as she seems to lag behind .
Phobias are not usually rational and it is perfectly reasonable for a Hyena not to trigger a Wolf phobia. Dogs might not even trigger it just due to the size difference.
Also explained much later in her companion quest :) (around the end of act2 i think?)
