There is so little else you can actually do in this game, it does not offer or react to choice in general, except for mainline quest choices about who you side with in bigger conflicts...
It's just not that kind of game where quest resolution other than 'kill everything' is anticipated or supported by the writers.......

No this isnt true at all lol. If you kill Ethel and wear the mask they are not hostile anymore, you can talk to the mask of regret. I just did that and saved her. And there are other ways to if you dont wanna kill Ethel either.
If you find yourself having to kill everything in the game you clearly missed the optional routs
Then the game bugged for me. I killed Auntie Ethel in the tea house before she escaped. Then went into her lair, put on the masks but I could not talk to the 4 masked people, I only had the attack option which was a shame. So I had to murder them all to at least get xp.
Sorry to hear, that must be a bug since that didnt happen to me and I killed her before she teleported to.
Man, I did something similar too.
I examined them, and saw they were under a spell from their mask. So I weant into the settings and was like, ah, I see, this is probably why we have that non lethal passive for.
SO i turned that on, went through that whole fight, not using spells, and ranged weapons and knock them all out which made the fight a lot harder than it needed to be, only for them to die immediately when i took their mask off.................

I think I know who your talking about, the ones in the blight town??? I failed that dice roll and they was like, nah.
Yup those are the ones but ofc I forgot to use the horn they gave me.
In early access you could spare them though. The gnome had dialogue but the others were too far gone, crazy though not hostile
else wear one of the welcoming masks for a very very short time or else you will be controlled, if all wear it, game over. this should let you move through the door.
