I'd add, "Fat" and "handsome" go together about as much as "nun" and "Assassin". And, let's be real: Statistically, fat is NOT attractive. I think the world will cave for LGBTQ but fat will always be unattractive.
Just cause WE (OP and I) find fatties attractive, doesn't mean the world does.
Fat could be attractive if it doesn't affect your face too much. Here, I said it first. Fat is only unattractive if it leads to 2 chins, hanging cheeks, fat skin around eyes and other deformities of that sort.
Now, if you remove all the magical elements from the game, you would have essentially a medieval themed game. And historically speaking, those who were not landed gentry or traders would have had to live a subsistence life, growing their own food, far removed from the riches of a nobles banquet hall. They would have more than likely a marginally higher caloric intake that expended in their daily workloads.
Famously, English king Henry VIII packed on a lot of weight after his tournament injuries rendered him unable to pursue his former active lifestyle. He ate rich foods and his caloric intake well overtook his physical activity. Thus he became obese.
Now back to the game, I can understand that folks want to create a character that reflects who they are as a person. But, for the most part, we live pretty sedentary lives, and our food intake is wildly different to the natural diet that medieval folk consumed.
Yes, it's a fantasy world where we should be able to be who we want to be.
But as some have stated before, the mesh geometry would be too difficult to model an infinitely variable types of armour and costumes to cater for those body types.
They can factor in bodytypes, its not rocket science, soon, we will see Modders bringing us glorious mods.
Be it face shape, height (of course with a limit, not a 2.5m dwarf or 0.4m half orc).
Kind of like Code Vein, TES, Fallout, Dark Souls... for instance.
