Companion approval

Are companion approvals pre-defined or random or depending on some factors/stats?
I'm new to D&D and really don't understand this system. In my first playthrough I'm trying hard to please Shadowheart as much as possible and sometimes her decisions just make no sense at all.

SPOILERS AHEAD
For example, we just met two guyz in the swamp arguing with an old woman and accusing her of being a hag who has kidnapped their sister. I've tried to take their side and offer my help to save the sister, in my understanding this would've perfectly aligned with a Cleric's way of thinking (Shadowheart is a Cleric). Other two companions approved this decision and she has disapproved. Why?
She made it clear that we should prioritize finding a cure for ourselves, but she doesn't mind or even approves plenty of other side activities. Take for example making fun of Redcaps in the same swamp, she approves that even though it's a waste of time.
So, why would she disapprove me attempting to save a young girl kidnapped by a hag?

It's predefined. But Shadowheart is a real finicky clown sometimes when it comes to this. She's supposed to be "neutral" aligned according to Larian, but in reality most of her dialogue is just good with occasional random outbursts of "you ok Shadowheart?".

Her being supposedly neutral makes this feel very random. I experienced this myself also. Fortunately it's mostly just her.

Well, thanks for the heads up, but I've used my previous save to turn this quest down because of her. If you're saying I will still be able to meet the hag / young girl then will definitely try to save her.

Thanks for letting me know it's pre-defined, I was thinking maybe some of my abilities and stats affect it in some way. Glad you have noticed Shadowheart is being random sometimes too :)
