Completely unplayable here too. Mine worked fine in EA and on launch up until today. Now it's extremely slow to get through the first loading screen to the main menu. I've verified my game files with no issues, restarted and fully shut down my machine, launched from the bin folder and uninstalled and reinstalled the game and nothing works. Drivers are all up to date as well. As soon as it gets passed the Larian logo screen and the nVidia screen, it just sits and spins the wheel at 100% extremely slowly. The background art takes forever to flip to the next one and when it does it takes like 30 seconds to switch. Extremely frustrating!

and I can play on ULTRA settings without any lag (though I have only played a few hours, so still in act 1).
but I would say go for fullscreen
check that your monitor driver is also up to date (not just via windows update or device manager but go to manufacturer website and check latest monitor driver there)
and check that your game is installed on SSD (that seems to be very important according to Larian Studios, though i have not tried on HDD myself so I don't know what difference it can make)
I use Vulcan and it works great (not tried DX11)
