Report a translation problem
Some said that restarting the game and toggling the reaction off and on fixes it for them temporarily but i havent managed to make it work yet so i just assume its bugged.
The reaction ONLY procs when you get hit. Not when you're attacked. So in order for you to be able to respond with Wrath you must first lose HP (or negate it with temporary hit points from something like Rally). Also they have to be within melee range of you. If they hit you from range you cannot react.
Also you only get one reaction a turn. So if you react with another skill or ability THEN you get hit by an enemy you will have already used your reaction and thus will not have the option of activating Wrath.
For example here's the actual description of the ability:
Wrath of the Storm
Also at 1st level, you can thunderously rebuke attackers. When a creature within 5 feet of you that you can see hits you with an attack, you can use your reaction to cause the creature to make a Dexterity saving throw. The creature takes 2d8 lightning or thunder damage (your choice) on a failed saving throw, and half as much damage on a successful one.
You can use this feature a number of times equal to your Wisdom modifier (a minimum of once). You regain all expended uses when you finish a long rest.
There are other skills and spell effects that can cause you to not be able to spend a reaction.
I'll give you what I believe is the most common scenario for not being able to react with Wrath. Let's say you're in a fight and there are two enemies near you. One enemy leaves you to run off and hit Shadowheart. When he leaves, he may proc an Opportunity attack because he's leaving your area of attack. Your character automatically attacks and does damage. After the enemy heads for Shadowheart the 2nd enemy near you attacks and hits you.
In this scenario you will not be able to react with Wrath of the Storm because you've already reacted once (to the other enemy) with an Opportunity attack.
Although that still leaves the issue of ranged (and other types of) attacks made in melee proximity - handbook mentions an attack being made within 5 feet (melee distance for this game I guess?) - not specifically that it has to be melee. There's also the matter of, for example. a charging minotaur passing by that doesn't knock us back (STR save succeeded).
I believe i found why the reaction seems to be inconsistent. First things first it triggers only against melee attack (not on miss) and the main reason why it doesnt trigger sometimes is simply because of the attacking creature size. I realized that it triggers consistently on goblins and small creatures that have a short reach. So, larger creatures will attack from further away resulting in your Wrath of the Storm reaction not triggering. I dont know if this is a feature or a bug but the quick fix is just geting closer to the enemy (literally hug them), it should trigger consistently.
