Dark Urge is a constant struggle against the urge to kill and mutilate anything. Sometimes you can resist, sometimes you cannot.
You'd need to find specific Dark Urge spoilers to know whether or not you can avoid killing a main companion.
"Not designed for" is incorrect. They said they do not recommend it as first playthrough (because you do not have control of all events and some might disturb you), however if you ask me, it's probably the best first playthrough you can do.
I'll probably run through the game multiple times before I try that kind of thing.
