This worked for me, however every time I touch the dpad, menus freak out and I need to disconnect the controller in order to be able to navigate anything. Using an Xbox One controller.
- disabled Steam Input for BG3 in Steam properties
- ensured BG3/Steam are off in Security & Privacy → Accessibility
- reinstalled the game
- connected controller before or during the game
- unpaired and paired controller again

that worked for me but each time you log into steam you have to do it again.
Today I decided to uncrate the Xbox console and *update* the controller... It worked.
I didn't change any other parameter and was able to successfully create a character and play for hours. If you've been struggling like I have, give this a try! Good luck.

Steam > Preferences > Controller > Enable Playstation Support
Game Options > Properties > Controller > Disabled Steam Input < critical
macOS Privacy > Accessibility Options: Steam and Baldur's gate enabled
Most importantly: do not use the controller until your saved game loads and you're in the game world - instead use the keyboard and mouse. If you click on a controller input at ANY time before this you'll trigger the bug.
Why this works: The bug we're all seeing is a result of errors in the game's Steam Input setting and not releasing the keyboard as an input device when a controller's present. The confusing bit (and why this solution works consistently), is the bug's only triggered in the game's initial menus, not when running in the game world.
In my testing I found you could essentially "trick" the game into behaving normally by toggling the PlayStation controller support and Steam input settings along with restarting the game multiple times, but that's just a big old waste of time. The work-around above works every time and doesn't require reloads.

@guagni.lorenzo - Sorry to hear this hasn't worked for you. I would stress we cannot use the controller until your save game has finished loading, and you must have Steam Input disabled under the game settings (gear icon on the games main page in Steam). The combination of settings above and process has worked without fail for me, I hope with a bit of experimentation it will for you as well.
I can confirm this didn't solve the problem for me yet.
>Click the gear on Baldur's gate or right click the game and select Properties
>Click General, uncheck "enable the Steam Overlay while in-game." If this is greyed out and turned off go to the next step.
>Click Controller and disable steam input.
>If the "enable the Steam Overlay while in-game." was greyed out click Controller General Settings underlined above the disable input option.
>Enable Steam input for playstation controllers
>Click in Game option above controller. Toggle on "Enable the steam overlay while in-game"
>Return to General settings page. Toggle "enable the steam overlay while in-game" on.
>Restart Baldur's Gate 3
>Toggle from controller and keyboard and mouse. If you can toggle back and forth it has worked. Double check by going to Baldurs gate 3>main menu>multiplayer>create new game. You should see that you occupy only one controller.
>If all of this fails go to mac>system settings>bluetooth>(your controller name)>i beside the controller name>game controller settings> ">" next to controller info> erase controller settings.
>Restart BG3

Turn off the controller and turn on again. Problem solved!

Tried literally everything here.
What helps for me:
Go to Windows PC
Install Xbox accessories via Microsoft Store
Update xBox Firmware controller via cable.
Forget Device on Mac, Reconnect via bluetooth
After that, controller works well.
So its not Larian bug, its Mac-xBox connection problem, so it will never be fixed with a patch.
