Western usually refers to ye ol Cold War nation splits, ie, Nato vs Warsaw Pact/Communist China.
Alternatively, it is a modern update to Occidental vs Oriental, which was typically a Europe/Asia divide (and got weird south of the Med).
Depends on who is saying it. If I see someone from Japan say it, I immediately assume they are referring to America for a long list of reasons.
