DnD doesn't have "vampire forms." Vampires more or less look like normal people of their race. They can turn into clouds of mist or bats to aid in escaping a battle, but there's no Elder Scrolls - style "werewolf, but grey, hairless, and leathery" form for vampires.
As for werewolves, while there was originally a werewolf companion planned, they got cut during development. Vampire mechanics exist in BG3 because Astarion is a vampire, but without a werewolf companion, werewolf mechanics seem unlikely.
Werewolves in general? No idea. I'd imagine unlikely, but maybe? I don't think you'll be able to become one anyway
I don't think you'll see a werewolf.
Doesn't really matter. It's kind of like splitting hairs here for the OP's question. Vampires are in, regardless.
That remains to be seen, since we don't have the full game in our hands yet.
But better to expect it to not be there and be pleasantly surprised in any case.
Anyways I'm sure a modder will add the ability to transform and control one when the game releases, there was a mod I remember not long ago that had it for the early access version.
I've always wanted to make a monk/werewolf character myself, so I'll probably be downloading that mod when the game releases.
A werewolf can transform at will and use werewolf attacks, plus their hearing and sense of smell gets a buff, there's also different types of werecreatures like werebears, weretigers and others. How exactly it works depends on each individual game though, the one time I experienced it my character lost control and tried to kill his friends so I made sure to get rid of it as soon as it was possible, while another party member who had also gotten bitten decided to instead try to learn how to control it, which worked.
If you're saying in general, a werewolf I can't say, but you certainly can play as a vampire spawn origin character if you wish. They'll be fighting to break free of their master's control and possibly become a full fledged vampire themselves.
