Also some manufacturers put a physical switch on their nvidia cards (probably amd as well) to toggle between "quiet" and "performance" (coolers are running louder) modes. I would try to flip that switch and see how it is going to affect temps.
Common things that can limit GPU usage.
1. FPS limit of some kind. V-SYNC, Direct FPS limits etc limit the maximum FPS required from GPU. This in other hand eases the load. This is the only reasonable limitation for user to set. With 120 Hz LG oled TV you simply do not need more than 120 Hz. Or 144Hz monitor with G-SYNC (or freesync or other VRR) This is the only limitation I ever use.
Now if limitation like this is are in use. Some game can go to 120 fpswith 50% GPU usage (maybe your elden ring) and some other does not even reach 120 fps with GPU working MAX (this game). This is the case for me most of the time. 4k is so heavy that GPU is always trying MAX.
2. CPU is not powerful enough. In case where FPS game can produce is limited by CPU not feeding enough data to GPU. Then CPU is saturated and doing MAX, but GPU does not have to do that.
3. Some software or computer issues.
- PCIE bus configured wrong
- driver issues
- etc
These are things to look for if points 1 and 2 are not in effect and you cannot get GPU to the MAX.
GPUz shows:
GPU temt 90 with hot spot 93
If i switched to Vulcan:
GPU temt 80 with hot spot 92

Wow, mine (3070ti) is steady at a even 60c, CPU (i9) around 50c. 103c seems crazy high.
103c is crazy high for that card although technically under throttle limit. I'm running a 6700XT which is nearly identical and it sits at 54c in this game. I do have fps capped to 60 and my case fans set to 50% while playing this game though because without doing that I was running low 70s. OP likely did nothing to counter the temps, but with temps that high I'd say they either have horrific airflow, faulty GPU fans, or an issue with GPU paste.
My 7900XTX hotspot goes up to 105C on 2800MHz overclock...

AMD does not "run hot" stop repeating the mantra from decades ago. AMD processors are in fact the most efficient as per today, obliterating Intel.
And AMD GPUs have the best performance per watt ratio out there, for example, check out the RX 6600.
You can play BG3 in Ultra at 1080p and 1440p at 60FPS or more, without performance issue, with a 200$ 6600 GPU. And, it uses 132W.
Just make sure your GPU fans are actually running / before there's a problem.

Moving on from the teenagers with more opinions than they have IQ: I will say Baldur's Gate 3 sparks my temps as well, hence why Im here. Quite surprising for a game that doesn't seem that intensive compared to other games I run without issue and at max settings, say GTA, Battlefield, Elden Ring, etc etc. Keeping in mind, when I say it runs hot, I mean it runs hotter than expected, but its not alarming. Temps remain within operational regions (Between 50C to 70C)
I did throttle back my FPS slightly which seemed to have helped a little but again, I feel I as though I shouldn't have had to. It may just quite simply be the fact that BG3 isn't that well optimised for AMD cards.
As mentioned above, recommend throttling back FPS, increasing fan speed and if possible, give your rig a good dusting. Remove any airflow blockages and reapply thermal paste. If you are still having alarming problems after that, then you may need to consider improving your cooling system outright which is the expensive option, unfortunately.
