Pierre is in F19
He will NOT join you if any of the following things happened
1) Allowed his father to be lynched on Ernie Island
2) Did not give the Ernie villagers the Machinegun from the bunker
3) Did not train some militia in Ernie village
4) Killed Bastien
Every time I recruited him I had also killed Bastien , so it can't be a hard requirement not to. But for me it always happened at the fort, so could be that doing it in the starting sector has a different outcome.
No, Pierre joining depends of various decisions you made over the course of the game.
Not sure, there are 4 potential places to meet Bastien (3 on Ernie Island and later in the Refugee camp)
1) Starting sector
2) Outside the bunker (can be convinced to leave Ernie)
3) At the Fort (will fight you if he was not convinced to leave)
4) Refugee camp
If you take the right steps at encounters 1 and 2 you complete the 'Reform Bastien' quest. This is supposedly integral to convincing Pierre to join but I guess not if you killed him at the Fort and Pierre still joined.
Re the refugee camp, I don't think it matters what happens to him at this point.

I am not exact, on how many it takes but each time I was able to recruit him. I am sure if some of those things that he disapproves of you doing and if so, why would he want to join you but with that said he is after all, in his own country, so try to think of it that way and it all should work out.
