I'm not sure it would make the game 'harder', it would make it far more confusing.
'Hard' mode would be self-imposed rules, I think.
As in:
"Must take all starting tickets, deal 5 to each player."
"Each player has less trains than they should in a normal game."
"Double routes can not be claimed in a 4-5 player game."
"To claim tunnels, a player must turn in two sets of the colour required."
"Players may nominate a city to which an opposing player must claim to score points for a route." (Eg. I have LA-Miami. They nominate Calgary. In order to score points for the LA-Miami ticket, it must include Calgary in the route.)
Things like that would make the game harder, adding expansions together would make the game much more... thought intensive as opposed to specifically harder, I would say.
