Make sure you are pinning your windows and closing Edit Mode! If you have Edit Mode open you'll see the OVR Toolkit interface which also means you can freely move around windows. :)
(Window pinning is done through the Window Settings menu, one of the top slider options! A pinned window will show all the time, even with Edit Mode closed. This means you can have a chat window pinned so it's always visible and can't be interfered with whilst in game, then apps like Spotify only visible when toggling Edit Mode to quickly use them!)

Alright, this has helped some; But I've run into another issue.
When using the Index, there are no grip "buttons". Rather, a grip button input is sent every time you squeeze the controllers.
When actively playing an intense game, the player is frequently squeezing the controllers without thinking about it. So even if a window is pinned and edit mode is turned off, I find myself frequently re-enabling edit mode without thinking about it during regular gameplay. This, in turn, makes my pinned windows vulnerable to getting lost.
Is it possible to disable grip-button edit mode enabling all together and re-bind it to something else? I noticed that double tapping both A buttons also works. Is there a way to restrict edit mode to just that?

i also tend to squeeze the controller sometimes and accidently opend the edit many times,
nearly never happened with the buttons,
could he have options to adjust the buttons on the hand controller?

Yep, this is already an option.
Just remove the 'editmode' bind from the grips from within the SteamVR 'Old bindings UI'.
(SteamVR Dashboard > Settings > Show advanced settings (bottom left) > Show old bindings UI ('Controllers' menu) > OVR Toolkit)
Sorry it doesn't use the new UI, SteamVR don't include overlay applications on it for whatever reason. :)
