No idea why they didn't call it codex and why they didn't put a more direct access (to at least feel it's related to the game content and not something "outside" the game like the settings). Particularly as it contains really useful info that is needed to play and is no present anywhere else, like what is the use of the power cells on the various modules that can receive them.
I quite struggled to find it, looking in the keybinding menu to see which key opens the codex, searching thoroughly through the hub and the ships, to finally conclude it wasn't in the game yet. Then I accidently found it, while I was no longer looking for it.
There's a good point that the Achievement is hard to decode - especially that it read 0/1 while relating to multiple entries. We could also put the locked entries into the Data Archive as encrypted entries so it's easier to track progression.
And rewording the description to "[...]in the Logbook's Codex[...]"
When we're searching for the "codex", seeing a word different than "codex" doesn't make us jump to the conclusion "that's were the codex is" but is more likely to make us think "not here either" and we start looking elsewhere.
Particularly as we're already in the escape menu, so "outside" of the game world (in the escape menu, I expect everything related to the game that take place outside of the world of the game, out of my character reach, like the settings, not to find data collected by my character and "available" to them).
We don't necessarily go further when we already think it's not the right direction.
Simply having a key (L?) to directly open the logbook would make the codex way easier to find.
- At the moment, when you unlock a new lore piece in a run, if you try to unlock a new one it will unlock the same one (thus granting a duplicate)
- The behaviour above will be fixed in an upcoming patch: then, it'll be properly randomized
- Until then, if you want to grind this achievement, consider starting a new run after you unlock a lore entry in the current one

Can you explain if you get credit for the lore even if you fail/explode? Or do you need to return back? I feel like I've made no progress on this and now I realize it's because of this duplicate issue you mention..
Tough to swallow as I've had 3 20+ objective solo runs now that I feel were wasted effort chasing this post brain hotfix. But I also failed these attempts at some point due to laziness from not paying attention to reclaimer. So now I'm wondering if I ever got any lore credit for the long runs at all. My logbook shows nothing new to alert me. Very disheartening.
You get progress as soon as the celebration appears in your screen (mid-run) - at that time, you can already check out your new entry on the Codex (so don't worry about exploding right after! :D)
We expect the patch containing the fix to go live in the upcoming weeks, but until then if the achievement is your goal prioritize Ambush/Download/Raid and, as soon as you put the lore shard into the Astral Map, jump back to base and go again! :)

They can also be found in Raids. As to how many there are, I am not sure.
This appears to have been fixed as I got 2 different lore shards in a recent run.
how many are there? from the binary on the picture one could assume it will be 89 logs in total?
i wasted so much time on other games achievements in the past. but not sure if this is worth it with the progress i make.
I dont think its worth your time at the moment given the rarity of the lore shards and not knowing how many there are, I think the devs should make the lore shard an extra mission like the high priority target mission so at least your able to get 1 shard per run
for a while i had the feeling to get a new codex entry with each shard found, till recently. now i am getting duplicates.
So, are the "Brain in a Jar" entries bugged, or do they need special circumstances to be found? Or was my feeling absolut subjectiv so that all the entries are equally propable for each shard found. What distribution function is there?
First you have to be lucky to get a download or ambush mission, (i don't think i ever got something on a raid in like 30 hrs), then the mission needs to have the console, maybe the case for one in three missions. and than you need to be lucky to get a missing entry.
A garantuee to get a new entry on each shard would be good enough for me. Because thats too much RNG and will take houndreds of hours of just grind (start game, jump, restart if there is no download or ambush, check station, restart loop) otherwise. which is super boring. Getting this the normal way while playing a normal game loop is impossible.
