I have been searching for a long time.
Have I missed anything?

Enter the opened door, where the tree icon and will be 100%

Finish the trade quest and it shows you everything you missed on the map. If you have finished it already you're probably just missing a pixel of the map somewhere. Or you haven't gone through the final door yet. I think that small part before the boss also counts towards the percentage.

Is the final quest supposed to be never completeable? I have the same map as OP so I was thinking maybe I'm missing that quest completion. I have otherwise completed all side quests and 100% on all areas except for willow's end. I even completed the end boss twice.

That quest never technically completes, but it doesn't count towards your 100% map completion. You may have missed a bit of map or a small bit of Spirit Light somewhere.

Ah I found it, it was infact a small piece of map.

I had the same problem. What fixed it for me was going to the very top, to the top right spinnig portal thing, where one of the infected hearts used to be. I was not grayed out, so I stumbled upon it by luck. Either I did not take the portal when I was there or it was a bug, but going back there gave me 100%.
