possible bug - Midnight Burrows 4th keystone missing

As the title said, I have 4 missing slots ,but the counter stucked 3.
Anyone else?
I played 15 hours yet, how can I solve this? :D

Did you perchance already visit the Midnight Burrows and pick up one of the keystones? Because a few people have mentioned doing this, then finding out that they used it on another door somewhere else in the game.

I think Im at the end of the game, didnt see any closed keyed door. :(
But yes I thought this is the problem so searching right now.

i have this map for you to check and it shows that in midnight burrows (the one below this bell puzzle ?) has 4 door stones in it
