THE AGLET! (in all seriousness though, every end game player needs a pair of wings)
Aside from that there isn't really a best combination. It's all up to your playstyle.

Not sure about combinations, but the Ankh Shield is very badass accesory and it's pretty dang hard to get. It requires 11 items: a cobalt shield, obsidian skull, megaphone, nazar, fast clock, trifold map, vitamins, armor polish, blindfold, bezoar, and an adhesive bandage. only the obsidian skull is craftable, cobalt shield can be found in dungeon chests, and the rest of the items are 1% drops from specific mobs.

cobalt shield combines with the obsidian skull to make an obsidian sheild, the trifold map and the fast clock combine to make the plan, the vitamins and armor polish combine to make armor bracing, the adhesive bandage and the bezoar combine to make a medicated bandage, and the nazar plus the megaphone combine into a countercurse mantra. The armor bracing, countercurse mantra, the plan, medicated bandage, and the blindfold combine into an Ankh Charm, and the obsidian shield combines with the Ankh Charm to form the Ankh Shield. All crafting is done on a tinkerer's workshop.

The accessory grants: +4 damage, immunity to fire blocks, immunity to knockback, immunity to most debuffs.

I'd add lightning boots to the "every end game player needs" list but it doesn't necessarily have to be equipped at all times. It's very handy for getting around, especially on a large world. Otherwise it's entirely dependent on what weapons/armor you plan to use as to which accessories you'd want.

Wings, Lightning Boots, Ankh Shield, Master Ninja Gear, Charm of Myths. You will never need any other accessories (From experience, I have killed every boss so far without changing this set)

I currently have a fire gauntlet, fire wings, lightning boots, an ankh shield and master ninja gear.

Also, the Fart in a Jar. (Why? Just why?)

The only response that is needed, why not?

Honestly, most of the time my loadout is based on mobility not offense or defense. Currently it consists of Ice/Leaf wings (changes depending on vanity), Bundle of Balloons, Lightning Boots, Diving gear (eventually gonna be replaced by Moon Shell), and obsidian shield. I will occasionally replace the Diving gear with Lava Waders, or Star Veil, or Frozen Turtle Shell depending on situation. I can travel from one end of my large map to another without getting into a fight with a single enemy. Heck even my arena is based on mobility (and blocking lasers) I have blocks every 3 platforms to block lasers, but I can still move around smoothly. It's kinda fun actually. I almost never stop moving during a fight.
