My mirror has never disappeared, not sure what happened to you.
If you can't find another then you can always use an inventory editor to get it back (It's only cheating if you give yourself stuff that you don't already have or had.) BTW before being asked: Google/Bing/Search Engines exist.
That's what I was thinking had happened. I keep it off my hotbar so I don't accidentally use it when i don't want to. So I may have had it "in hand" when I exited the game.
I googled it, didn't find a hard and fast answer so I posted here, sorry if I offended your delicate sensibilities. BTW no one in their right mind uses Bing.

My guess is the search engines comment was in relation to inventory editors if you wanted to use one to replace your lost mirror. It certainly seems most likely that it's just a case of accidentally dropping it. That's part of why I do some last minute inventory management before quitting Terraria. I'm not likely to have accidentally left something in my hand if I was specifically organizing my inventory at the time.
As for Bing, Microsoft would certainly love for you to believe otherwise.
