Can someone tell me what parts I need to make it and which enemies drop the items to craft it.

Yes.

First of all you need a cobalt shield. They can be found in locked gold chests in the dungeon.
Fuse this with an Obsidian skull, crafted from Obsidian at a forge. You now have the obsidian shield

Next up is all the status protections.
Armor bracing is made with Armor polish from Armoured skeletons in hardmode and vitamins from corruptors and the floaty gross.
Medical bandage is made from bezoar, dropped by all hornets, even in normal and bandage dropped by werewolves (quite often) and angler fish (not so often)
The plan is made with a trifold map, dropped by mummies and fast clock dropped by mummies, pixies and wraiths.
Countercurse mantra is made with a nazar dropped by cursed skull inthe dungeon (can be gotten in normal) and megaphone, dropped by pixies.
Blindfold is dropped by dark mummies and slimers,
Mix all of these together to make the Ankh charm.

mix the ankh charm and the obsidian shield.
