Found this in the wiki page for Wall of Flesh.
The Wall of Flesh's spawn location is determined by where the Voodoo Doll was thrown in relation to the center of the world. Thus, you can get the maximum space to fight the Wall by reaching the edge of the world and throwing the doll toward that edge.
This also determines the direction it will face. Keep in mind being on the left side of the world will make it sweep from left to right, and the right side will make it sweep right to left when making your WoF arena.
Though if you prepare properly, you really don't need much room. Using any type of block you can make a bridge over the lava that you can run on when you fight. obsidian skin potions help and water walking as well.
This, exactly. The absolute safest way to fight it is by running to one end of your world and toss the doll in the lava there. Ideally, make some platforms to run on too (or some other alternative, especially if you don't have strong weapons yet). Your character's facing does not matter.
It's as simple as that. There's no randomness (unless I suppose you happen to drop the doll at the exact center of the world), it's all about where you drop the doll into the lava in relation to the center of the world.
Yeah, building a bridge is definitely a 'must' if you don't want to mine though hills and Underworld Towers with Water Walking potions. I was just referring to how the game calculates which side it will spawn from. Also, as to the "center of the world" part: Since the tiles are singular, the game will see how much of the doll item entity is over which lava tile, which is still pretty random, since you toss the doll to the side instead of throwing it straight down. If you stand still when you're at the center and throw it you should know which way it will come from.
