Max is 6 slots. It's only usable in Expert mode, meaning in normal you can't use the item nor 6th slot will be available to you. If you aren't able to use it in Expert mode and don't have 6 slots yet, then i'm not sure.

1. maybe it's expert mode only item and can't be used on normal worlds
2. maybe you already have 6 accessory slots for some reasons
3. if you don't sure how use it, try two ways - put in apper slots and use number keys to select it and use with left mouse key or try take with left mouse in inventory move cursor out of inventory and just left click
