It's a buff potion, lifeforce potion, I think. I'm pretty sure it's crafted from primsite (a rare fish from the hallows) and some other stuff. I don't fully remember. Just take a bottle of water to the guide.

The recipe is: 1 Bottled Water, 1 Prismite (Fish), 1 Moonglow, 1 Shiverthorn and 1 Waterleaf

You can craft it at a Workbench/Table with a Water Bottle placed on top of it
