You can actually use this to make it "safe" to explore and prepare the underworld. Kill your guide before you descend your Hellevator, making sure there's no housing in the world for him. Go down, explore and prepare the Underworld, and if the Guide's dead, it won't matter if a Voodoo Demon drops their doll into the lava. Then when you're ready to fight the wall of flesh, make sure the Guide respawns by giving him back his house, go back to the Underworld, and toss a doll into the lava.
The names of every NPC gets randomized. They still have the same function. If you kill a guide named Andrew, he'll drop a green baseball cap!
