if you turn your cloud off and fully wipe the game from your folders then uninstall it then you lose everything by doing a fresh install : P

Depends whether your worlds are saved on the cloud or not, while cloud is by default enabled, your worlds and characters are not by default saved on the cloud.

If you have them saved on the cloud then uninstall then they'll be fine, and
(as far as I'm aware this is how the cloud works anyways) will be accessible on every device you have your cloud synced to.

If they're NOT saved to the cloud, then you will lose them most of them time if you uninstall, you can save and backup your worlds and characters respectively through the locally stored files, which can be found here if you are running on windows 10 (presumably also 11):

Then worlds are stored in the "Worlds" folder and characters in the "Players" folder.
If you want to back them up I recommend just copying all the files you care about to another
backup folder. Worlds have 3 files, one main file and two backups, characters have 2 main files and a folder respectively, I recommend copying all files related to the world/character.

Then it's a simple matter of moving them back to their original places when you reinstall.

I would honestly just make a manual backup. Even after going though the process of getting everything onto the cloud, Terraria is known for deleting things stored on it, which includes off the the computer where they were originally uploaded, as it no longer has a local copy.

Best to not save to the cloud. It notoriously corrupts data, so if you want your data to be safe then it is better to copy the world and character files and keep them separate.
