I just lost A weapon.

I am pretty sure I didn't press T but if I did then I guess I lost it for ever. is there any way of reporting this kinda problem? how can I get it back?

Depends on how you "lost it." If you accidentally threw it away as you were logging out, it's gone forever and there's no way to retrieve it (other than using an inventory/map editor to add one to your character/map.) If you simply left it in a chest somewhere, then obviously if you can find it again you'll have retrieved it.

I've never actually experienced any bugs like that before but if you want the devs to hear about it, you'll need to post it on their official forums here: http://www.terrariaonline.com/forum/

The bug reports section would probably be the best place if you feel certain it's a bug.

it was the Death Sickle. I even reforged it in to a legendary.

I've managed to lose two rapid chainguns in the same way. I just replaced them, though as I have so darn many of them and more money than I know what to do with...

Only way to prevent this is checking your hotbar on regluar basis if you still have the items you use mainly. Yeah throwing away items with T can be tricky and you lose stuff that way. Probably avoid this by remapping T to a different button which you will never push ?

yeah i set mine to num7 which i never hit unless i want to drop something

Considering you can't craft a Death Sickle, replacing it would be more than a little difficult.

If you lost it through something stupid (such as dropping it somehow, forgetting you dropped it, and then saving and exiting the world), I would recommend going and using an inventory editor to get another one. If you lost it to death on Medium or Hardcore, then, well, ya lost it, but it was fair and square.
