you can just put it on, heal, take it off and switch it for another add-on. just put it on whenever you heal so it takes less time to heal again
I feel like constantly switching the philosopher stone during a boss fight might be very challenging (while avoiding attacks anyway).
Potion cooldown was implemented to prevent cheesing all boss fights by constant healing, which make fights boring and not challenging.
Finally, the recovery potion (normal mushroom + glowing mushroom + pink gel + bottle) have a lower potion cooldown (that stack with philosopher stone).
Also, learning boss attack paterns might increase your survivability.

Oh right, i forgot the game have the option to auto pause lol. Everyone have their own playstyle.
I don't think you can compare different games with different game mechanics and different goals. Some games are easier and more suited for casual players (i don't have an exemple here, but i'm sure we can search for one). Some games are much harder and try to target hardcore players (like dark souls). What looks "fair" to you isn't necessarly what is considered "fair" for others, and that include the devs. There is also the question about what "fair" mean to you: making the game much easier? to prevent obvious cheese? to give a good challenge with no hidden surprises?
It's not like the 15 health from mushrooms would be helpful in a boss fight, but can make recovering within like the first 5 minutes so much easier.
1 Minute is pretty much the enough time for you punishment of death before you can get more life for another shot or 2 with healing, the ones who dodge [or tank with specific setups] are awarded this way with longer life.
Pre hardmode is bit meh since you can deal away with a lot of enemies but in Hardmode 60 seconds/45 seconds cooldown makes a whole lot of sense.
