shrine either spawns or doesn't spawn (just keep making new large worlds until it spawns)
once it spawns
it drops the enchanted sword, or drops other sword

arkhalis doesn't drop from this underground shrine anymore (at least on PC)
you need to kill bosses until their treasure bags drop it

There are also uncommon instances of worlds generating without any sword shrines close to the surface which basically forces you to fish for Gold & Titanium crates which have a 2% drop chance for the Enchanted sword which is practically impossible to get without sinking lots of hours into the game since the luck mechanic doesn't affect what you get from crates.
So its better to just generate worlds & use TerraMap to peek and check for Shrines.
Who knows, peeking at the worlds you generate may give you your desired biome layouts in world generation.
Aaaand... Terragrim replaced Arkhalis as the 2% drop chance from sword shrines as of 1.4.
